The rotator cuff is made up of a group of muscle and tendons. The muscles and tendons help support and keep the rotator cuff in the socket of the shoulder. Furthermore, it helps the arms mobility by moving the arm from abduction (away from the body); adduction (toward the body); flexion (moving the arm forward) and extension (moving the arm backwards). There are four muscles that make up the rotator cuff. These muscles (also know as SITS) are: supraspinatus, infraspinatus, teres minor and subscapulans.

The supraspinatus is a small muscle and it helps the deltoid muscle move the arm in abduction. This causes the humerus to move toward the glenoid fossa, so it will provide stabilization of the scapula. The supraspinatus muscle is one of the rotator cuff muscles prone to being injured. An injury to the supraspinatus muscle can cause the muscle to become inflamed or even torn. Such injuries can come from athletes, lifting things, and falling on the shoulder. Also, as we age our bodies degenerate and this causes our bodies to become weak and our bones brittle. This causes our bodies to be injured easily, especially in older adults. Repeated movements can irritate the tissue and cause bursitis (inflammation of the bursa). This can be very painful and limit the mobility of the arm. Supraspinatus tears can be partial or full tears. People often do not get their full range of motion back after serious injuries such as a tear.

The infraspinatus muscle is a thick muscle with a triangular shape. The infraspinatus muscle connects in the middle of the posterosuperior and anterior parts of the rotator cuff. The main function of this muscle is to rotate the humerus externally and stabilize the joint. The infraspinatus muscle is also prone to injury from trauma or over use. Activities that can cause possibly because injury includes sports such as baseball and repeated motion that can cause stress to the muscle. Injury to this muscle can be strained or torn, causing pain to the arm and limiting mobility.

The teres minor muscle is a long and narrow muscle. When the shoulder moves, the teres minor stabilizes the humerus in the glenoid fossa. The teres minor is at the lateral edge and next to the posterior surface of the scapula and it is involved in the external rotation of the joint. The teres muscle is the second most likely to be injured by activities such as: painters, baseball players and rock climbers. These activities can trigger the muscle causing pain in the shoulder blade. Depending on the trigger point, injury and pain can be acute or chronic.

Finally, the subscapularis is a triangular muscle that fills the subscapular fossa. During shoulder movements, this muscle stabilizes the head of the humerus in the glenoid fossa. It is placed into the lesser tubercle of the humerus and front capsule of the joint. The function involves the internal rotation of the adduction. The subscapularis is the most frequently injured because it undergoes the most force (Morgan, 2016). Most injuries are caused by falls or degeneration disorders such as rheumatoid arthritis. Since this muscle stabilizes the shoulder, injury can cause loss of mobility.

Rotator cuff injury occur more often in overhead activities. Rotator cuff injuries that are acute can sometimes heal depending on the injury. Chronic injury may need more medical attention such as surgery. Our bodies wear and tear as we age and as this happens, our joints end up bone on bone and eventually replacements could happen. Shoulder replacements mostly occurs in older adults. Some injuries can be prevented by exercising and keeping the muscles strong, stretching to keep muscles flexible and applying ice where the pain is located.

Critical Thinking

The rotator cuff is very important because it helps us accomplish daily activities and injury can limit the mobility to do those activities. I work as a rehabilitation technician and I have spoken with quite a few patients with shoulder replacements or rotator cuff injuries and they all say, “you never know how much you use or need something until you cannot use it or do things anymore”. Some patients lost their full range of motion and there are some that were able to gain full range back. Knowing the muscles, functions and activities that may cause injury to the rotator cuff can help someone understand how important the rotator cuff is in the shoulder and possibly help others to prevent injuries to the rotator cuff.
